Seguridad y eficacia clínica de la cinta vaginal libre de tensión (TVT-O) en el tratamiento de la incontinencia urinaria de esfuerzo femenina

RESUMEN
Introducción. La incontinencia urinaria (IU) es la pérdida involuntaria de orina objetivamente demostrable, la más común es la incontinencia urinaria de esfuerzo (IUE). El manejo es fundamentalmente quirúrgico con una gran variedad de procedimientos. Actualmente los procedimientos de mínima invasión son las mejores vías terapéuticas debido a la pronta recuperación, rápido retorno a las actividades, mínima morbilidad, bajo costo y sencilla realización. La cinta vaginal sin tensión (TVT-O) ha demostrado tasas de éxito a corto y largo plazo comparable con otros procedimientos. El objetivo de este trabajo fue determinar eficiencia y seguridad de la
TVT-O en el tratamiento de IUE, así como evaluar el estado de continencia urinaria posterior al procedimiento e impacto que tiene IUE en la vida cotidiana.
Material y métodos. Estudio no aleatorizado, con diseño de pre antes y después, que incluyó a pacientes de la Consulta Externa de Ginecología diagnosticadas clínicamente con IUE. Se evaluó el grado de severidad de la IUE y su impacto mediante dos cuestionarios cortos sobre disfunción urogenital (UDI-6) y de impacto de la IUE en la calidad de vida (IIQ-7).
Resultados. Se incluyeron 50 mujeres, 14% con disfunción urogenital leve y 86% con moderada. En el seguimiento se observó mejoría tanto la IUE como en la calidad de vida.
Conclusiones. La TVT-O en el tratamiento de la IUE es una técnica sencilla, segura y eficaz que mejora en un porcentaje importante la calidad de vida de las pacientes.
